National Health Insurance
Since health insurance is expired, you need to choose either carrying on arbitrarily or joining the National Health Insurance. (However, those who can carry on arbitrarily provided that they have joined the National Health Insurance for a certain period. For further information, please contact the health insurance association that you joined or Kariya Social Insurance Office (1-401 Kotobuki-cho Kariya Phone: 21-2111) if it is the government health insurance system.
If you desire to join the National Health Insurance, a certificate showing that you lost health insurance (for example, leaving certificate) and a personal seal.

National Pension Plan
It shifts from the Employees’ Pension Plan to the National Pension Plan (no concern to those who are at the age of 60 and older). To perform the procedures, a leaving certificate or a letter of separation, a pension book and a personal seal are required. Those who have a dependent spouse, his/her pension book is also needed.

Final Income Tax Return (following year)
A corporate employee cannot make year-end tax adjustments if leaving a job in the middle of the year. Income tax deducted from salary can be reimbursed by filling out a final income tax return in some cases.

Aid for Geriatric Care and Welfare-related Medical Care
For those who receive aid for welfare-related medical care, including medical care for the elderly, infants, single-mother family and the handicapped, and change health insurance after leaving a job, please submit a notification of change of health insurance.

Social Welfare

School Expense Subsidies
In some cases in which a person resides in the city of Chiryu and their elementary and junior high school-aged children are unable to go to school because of a guardian’s financial reasons, they are qualified to receive aid for food service, school supplies, and so forth. An application form and related documents are available at elementary and junior high schools.

Unemployment Benefits of Employment Insurance
To follow the procedures for receiving benefits, a letter of separation for person insured under employment insurance, a savings passbook, (excluding the one at the post office) and so forth are required. For further information, please contact a job center.
