Physically disabled persons’ certificate

The Physically Disabled Persons’ Certificate is issued for a person with physical disability who is regarded as having a disability stipulated in the “Law for the Welfare of Physically Disabled Persons.” Disabilities are classified into grades 1 to 6 according to severity in descending order. They are further classified into the following categories according to the details of disability: sense of sight, sense of hearing, balance function, phonetic function, language function, masticatory function, orthopedic impairment, inside of the body (heart, kidney, lungs, bladder, rectum, small intestine, immunity). Those who have the certificate are qualified to receive various welfare services.

• Photograph of the person’s upper body (4 cm long and 3 cm wide)
• Doctor’s certificate (There are application forms at the counter in the city hall)
