Treatment certificate

This certificate is necessary for the city to provide consistent guide and consultation for persons (children) with intellectual disabilities, and various support systems such as admission to a care facility more efficiently.

Disabilities are classified into the following categories according to the severity: severe (A), moderate (B), and mild (C).

You need to be qualified by Nishimikawa Counseling Center for children and persons with disabilities (Phone: 0564-27-2889) to obtain the certificate (for children under 18 years old, Kariya children counseling center, Phone: 0566-22-7111).

Please make an appointment prior to your visit to the Welfare Division (for 18 years old and more) or the Children Division (for under 18 years old) as by advance reservation only.
