The infants etc. before the end of first grade of elementary school (until the first March 31 after reaching the age 7)
Details of subsidies
All amount of self-pay at the health insurance treatments (The subsidies do not include the costs excluding the health insurance treatment, such as expenses of meals and containers.)
Procedure
- Health insurance card (the one where the infant name is described)
- Seal
The usage
- For the persons who went through procedure, the medical care certificate for infants etc. (effective only in the prefecture) are issued.
- When the health insurance card and the medical care certificate are shown at the counter of medical institutions, the payment for the self-pay at the health insurance treatment is exempted.
Medical costs refund (in the case of seeing a doctor outside Aichi Prefecture)
- Receipt where the details of medical costs are described
- Health insurance card
- Medical care certificate for infants etc.
- Seal
- Bank account number whose name is pensioner’s (except Japan Postal Savings)
