For who is receivable
- The person who is 75 years old and more (the person who was born on the days gone by September 30, 1932, is continuously receivable.)
- The person who is over 65 years old and disabled to some extent (from grade 1 to 3 and part of Grade 4 of physically disabled persons’ certificates, and grade A of treatment certificates, and from grade 1 to 2 of health and welfare certificates for mentally disabled persons)
Details of subsidies
- The copayment (self-pay at the health insurance treatments) are as below corresponding to the income.
*Income earners as much as active workersּּּּּּ30% burdens of the medical costs
*Generalּּּּּּ10% burdens of the medical costs - If the copayment is made over the limited amount of self-pay (monthly), the exceeded amount will be refunded as the high cost medical care.

- Income earners as much as active workers (30% defrayers) mean that the taxable income for the city inhabitant tax of the persons or the seniors (70 years old and more) in the same family unit is 1,450,000 yen or more. In addition, if the sum of the incomes of the persons of over 70 years old and for aged insurance is under some extent (In the case of the family who has single senior, the yearly income is under 3,830,000 yen, in the case of the family who has 2 seniors or more, the yearly income is under 5,200,000 yen.), the family should be the 10% defrayer with such application.
- The low-income II means that the city inhabitant tax on the person and all of the family members are nontaxable.
- The low-income І means that the city inhabitant tax on the person and all of the family members are nontaxable, and their incomes are 0 yen (income from the pension are under 800,000 yen).
- The amount of 44,400 yen inside *( ) is the one after fourth of the patient’s self-pay if the high cost medical care’s refunds are made 4 times or more per year.
The application is necessary for applying for low-income І and II.

- The city sends the notification to the persons who are receivable.

- Bank account number whose name is pensioner’s (except Japan Postal Savings)
- Once the application is made, the refund is transferred to the applied account at every high cost medical care. (If the account is changed, please apply again.)
